The 2003-2004 John M. Chemerda Lectures in Science, by Dr. John F. Nash Jr. have been video taped for archive purposes. These tapes are made available for duplication through Media Technology Support Services at Penn State. All video duplication requests need to be in writing. There is a nominal charge for duplication. An estimated cost (for the video dubbing) is approximately $11.00 if supplying your own blank tape, or approximately $15.00 if a blank tape is to be supplied by MTSS. 5) PHONE: A phone number where we can reach you from 7:30 a.m.to 4:30 p.m. (Eastern), Monday through Friday.

6) DEPARTMENT: The department that you work for, or the department that is paying for the dubbing.
(If no department, leave blank)

7) EMAIL: Your email address. (If none, leave blank)

8) PAYMENT METHOD: Who will be paying for the dubbing? A Penn State Department, or will you be paying with your own money.

9) PICK-UP LOCATION OF COMPLETED ORDER: In which building would you like to pick up your order when it is completed.

10) TITLE: Title of the tape (if there is one). Use one line for each title.

11) LENGTH: Running time on the tape that you want copied? Please give an approximate time.

12) # COPIES: How many copies of each tape should our office make of your tape?

13) COPY SHOULD BE: If you bring in several tapes, would you like them all recorded onto one tape, or each title recorded onto its own tape.

14) WILL CLIENT SUPPLY BLANK TAPE: Did you bring in blank tapes for our office to make copies, or is our office supplying the blank tapes? (There is a charge for blank tapes provided by MTSS.)

15) TRANSFER FROM: In what format is the original tape recorded? For example, if your tape was recorded in the USA the format would be NTSC. (If you do not know the format, leave blank.) VIDEO DUBBING ONLY

16) TRANSFER TO: In what format would you like your copies to be recorded? For example if you want it to be viewed in Austria, the format would be PAL. VIDEO DUBBING ONLY

17) COUNTRY TO BE USED IN: If you do not know the format, in what country will this tape be viewed? VIDEO DUBBING ONLY

18) RECORD COPY AT: How do you want your copy recorded. Standard play makes a higher quality tape. If you are going to edit this tape standard play records the best quality. If tape is longer than 2 hours and you want it recorded on one tape, it will need to be recorded in extended play.
